Client records are maintained within JaneApp, which is a PHIPA-compliant platform. Email correspondence from clients is also retained, as it may contain clinically relevant information. Website contact form submissions are stored within Squarespace and are retained indefinitely as they may contain information relevant to the therapeutic relationship.

All email and cloud storage accounts used in the course of this practice are protected with two-factor authentication.

Client records are retained for a minimum of 10 years following the end of services, in keeping with CRPO standards.

Your information may be shared with third parties only in the following circumstances:

Clinical supervision: Jacob Damelin meets regularly with an accredited clinical supervisor. In the course of supervision, your first name and relevant clinical information may be discussed. Your supervisor is bound by the same commitment to confidentiality. This is disclosed in the consent form you sign at the start of therapy.

Insurance and direct billing: If you have requested direct billing, relevant information will be shared with your insurance provider to process your claim.

Referring professionals: Information is only shared with a referring physician or other health professional with your explicit consent.

Legal obligation: Information may be disclosed without your consent if required by a court order, or if there is a reasonable belief that you or another person is at risk of harm, or that a child is in danger.

Third-party platforms: JaneApp and First Session process payments on behalf of Jacob Damelin Therapy. iCal is synced with both platforms for scheduling purposes. Each of these platforms maintains their own privacy practices and security standards.

Your information is never sold, rented, or shared for marketing purposes.

You have the right to:

• Access the personal information held about you

• Request a correction to any information that is inaccurate or incomplete

• Request that your information be deleted, subject to legal retention requirements

To make any of these requests, please reach out through the Contact page. Requests will be acknowledged within 10 business days and fulfilled within 30 business days where possible. If a request cannot be fulfilled in full, for example, where retention is required by law, you will be informed of the reason.
